This is a simple questionnaire grid that has a number of questions that could easily be adapted to suit someone's needs. The grid set currently has 'grid commands' on each page that read the question out so these would need changing if you want to change the questions. A row of 'quick access' numbers across the top of the screen allow for a quick jump to a specific question in the grid if there are too many to go through in one go.

A 'send to wordpad' button on the final grid jumps to a single button computer control page which automatically launches WordPad. Accessing the button on this page will paste the text from the questionnaire into WordPad, automatically save the file with the name 'Questionnaire' along with today's date and then close WordPad and jump back to the previous grid.
